File a Claim

Instructions for filing a claim 

1. Please complete in entirety the claim form and sign at the bottom.
   Click here to download the claim form.
2. Enclose receipts and proof of payment for all items for which you are requesting reimbursement.
3. We will need a copy of your e-ticket, hotel reservation cancellation and conference registration material indicating cost and amount paid to register and whether you received any refund or credit towards a future registration.
4. If we receive all of the necessary information with your completed and signed claim form, the entire process from date of receipt by us to issuance of your check will take less than 30 days.
5. If there is an issue with your claim, we will contact you either by phone or email advising you of what we need.
6. Please do not contact our office until the 30 day period expires.
7. Office hours are 8am to 5pm, Central time.
